Request a Dog Licence Change

If you need to make a change to your dog licence information, please submit a request through this form. Updates to dog licence information include:

  • Contact information (email, phone number, etc.)
  • Address
  • No longer needing a dog licence (moved, deceased dog, etc.)
  • Providing veterinary proof that your dog is sterilized (spayed or neutered)

For changes to ownership additional documentation is required. Please contact our office for additional instructions. If an owner has passed away, please provide a copy of their death certificate.
